New Patient Registration
If you live in our practice area and would like to register with us, please complete one of our registration forms which are available from any of our receptions at any site. You can also download the registration form HERE.
The form will allow you to state which practitioner you would prefer however, you will be registering with the practice rather than with an individual GP.
You will also have to complete the following forms in order to register and provide the practice with other information necessary for your care and treatment:

All of these documents are available from any of our reception staff.
Once you have completed the registration forms you are required to attend a New Patient appointment with one of our Health Care Assistants. The appointment takes around than 20 minutes. It is important that you attend as it helps the practice team to sign post you to all of the services available to you based on your health needs.
Change of Address or Telephone Number
If you move house please check that your new address is still within our Practice area.
We cover a large Practice area across and around the City and you may find it more convenient to come to one of our other two surgeries – just ask the receptionist. It's much easier for us to move your records to the other Surgery than for you to start looking for another Practice!
